Please supply date/rain date, location (street and cross
street); name and address of all residents and name/phone number of party
organizer.
All residents must be notified - 75% of residents must approve.
Sample petition forms are available at the Village office.
The Block Party Petition may now be down loaded filled out and dropped off at the Village Hall between 9:00 AM and 4:00 PM
